The patient is a 5-year-old healthy left-handed female with a mass on the right dorsal hand. The mass has been noticed for 5-6 weeks and is slowly increasing in size. The patient has difficulty making a fist but does not have any other symptoms such as lymphadenopathy, weight loss, or constitutional symptoms. Initial radiographs show an expansile, predominantly lytic lesion centered within the third metacarpal shaft, suggesting a possible aneurysmal bone cyst. MRI findings confirm the presence of an aneurysmal bone cyst in the third metacarpal diaphysis. The next step in the management of the lesion would be an open incisional biopsy or curettage with allograft bone grafting. The patient undergoes curettage with bone grafting, and the pathology confirms the diagnosis of aneurysmal bone cyst. Follow-up visits are important to monitor growth arrest of the metacarpal physis and to check for recurrence. Two weeks, one month, and 3.5 months after the surgery, the patient is doing well with no pain or issues with the surgical incisions and is back to regular activity without difficulty.
