A systematic review and meta-analysis compared the outcomes of cast immobilization versus percutaneous or miniopen screw fixation in the management of acute, undisplaced scaphoid fractures. The review included 10 studies, with a total of 376 patients. The patients were divided into two groups: group 1 received cast immobilization and group 2 received percutaneous fixation. The average time to return to work was 77 days for group 1 and 46 days for group 2. The average time to radiographic union was 79 days for group 1 and 44 days for group 2. There was no significant difference in complication rate between the two groups. The study concluded that percutaneous fixation of undisplaced scaphoid fractures has union rates comparable to cast immobilization but with faster return to work and time to union, without a significant difference in complication rate.
