This study compared the outcomes of open reduction and internal fixation (ORIF) and total elbow arthroplasty (TEA) for women over the age of 65 with intraarticular distal humerus fractures. The study included 24 women who underwent surgical treatment and had at least 2 years of follow-up. The main outcome measures were the Mayo Elbow Performance score and the need for revision surgery. The results showed that ORIF had a 75% excellent-to-good outcome, but 25% of patients required conversion to TEA due to fixation failure. In comparison, TEA had an 91% excellent outcome and no patients required revision surgery. The study concluded that TEA is a viable treatment option for distal humerus fractures in older women, especially in those with comorbidities such as rheumatoid arthritis, osteoporosis, and conditions requiring the use of systemic steroids. However, longer-term monitoring and larger studies are needed to confirm these findings.
