Pdf Summary
Intra-articular fractures of the distal radius are challenging to treat due to various factors, including the complexity of the injury and potential complications. No single implant or technique is appropriate for all fractures, so surgeons should be familiar with different options. Displaced fractures of the articular surface have a worse prognosis than extra-articular fractures. Surgical stabilization is difficult due to various reasons such as close association with surrounding ligaments and tendons, poor bone quality, and potential complications. The article outlines the anatomy, classification systems, and treatment options for these fractures. Shearing fractures can be treated with plate and screw fixation, while compression fractures can be treated non-surgically or surgically depending on displacement and stability. Preoperative planning, imaging techniques, and surgical techniques for restoring alignment are discussed. The goals of treatment are to restore range of motion, forearm rotation, and grip strength while minimizing complications.

The study focuses on surgical treatment options for these fractures, highlighting complications associated with external fixation devices such as nerve injury, pin infections, and scarring. The use of plates applied across the wrist for traction is discussed as an alternative to external fixation. Non-bridging fixation and percutaneous Kirschner wire fixation are mentioned as treatment options for certain types of fractures. The use of bone grafts and substitutes, as well as advancements in tissue engineering and growth factors, are also discussed. Internal fixation and its complications like tendon irritation and rupture are mentioned. The study concludes that surgical fixation can lead to good results but there are still areas of controversy and dissatisfaction with current treatment methods.
