An MRI confirms an expansile lesion with focal cortical breach and a small soft tissue component, suggesting an aggressive cartilaginous neoplasm. Because pathology may not be conclusive, due to similarities between enchondroma and low-grade chondrosarcoma, the lesion is excised with the placement of a structural intercalary iliac crest allograft. The pathology report confirms the diagnosis of chondrosarcoma arising from a pre-existing enchondroma. CT chest and bone scan are negative for metastasis, and the patient is advised to undergo regular monitoring with imaging.<br /><br />Overall, these two cases highlight the importance of timely diagnosis and appropriate management of malignant bone and soft tissue tumors.
