Pdf Summary
Skin and nail conditions can provide important clues to the diagnosis of systemic conditions that can affect the joints. They can also affect wound healing and surgical outcomes. This chapter covers common skin conditions that hand surgeons are likely to encounter and need to recognize.

One common condition is hand eczema, which is characterized by erythema and scaling of the palms. Pompholyx, or dyshidrotic eczema, presents with vesicles on the fingers and hands. Treatment involves topical steroids and skin care measures, such as applying thick creams, wearing gloves, and avoiding certain products.

Contact dermatitis, caused by an allergic or irritant reaction, presents with erythematous, edematous plaques that may become vesicular. Patch testing may be necessary to identify the cause. Treatment includes topical steroids and avoiding the allergen or irritant.

Psoriasis presents with plaques, scaling, and nail changes. Treatment depends on the extent of the disease and can involve topical steroids, calcipotriene, tazarotene, and phototherapy. Severe cases may require systemic treatments, such as retinoids, methotrexate, cyclosporine, or biologic agents.

Warts, caused by HPV infection, present as rough, hyperkeratotic papules. Treatment options include topical agents, cryotherapy, and intralesional injection. Some lesions may require biopsy to rule out malignancy.

Actinic keratoses are precancerous lesions that present as scaly papules in sun-exposed areas. Treatment involves cryotherapy or field therapies, such as topical chemotherapy or immunomodulators.

Other conditions covered in the chapter include lichen planus, granuloma annulare, connective tissue diseases, pyogenic granuloma, neutrophilic dermatosis of the dorsal hands, clubbing, and onychomycosis. Treatment options vary depending on the condition and may include topical or systemic medications, surgery, or other interventions.

This chapter provides hand surgeons with valuable information on recognizing and managing common skin conditions that can affect the hands.
